How many gal is your tank? Do it slowly - just replace the top up with saltwater at 1.024 or the desired salinity

Most LPS with sweeper tentacles need a wider area than it's base size. At night (brain or acans) can send out very long tentacles that will sting...
also your salinity is very low imo. should be around 1.023 to 1.026 - 35ppm
Your nitrate's are high, are any other corals reacting? what is your lighting? can u see the foot? or has it moved? like i said tho just watch it,...
