The anemone will look flacid and lifeless, it's tentacles will not be sticky, and it will look that way for more than 12 hours. Sometimes when an anemone looks poorly, it is just "pooping" and will recover overnight.
Also, check your water parameters. Anemones are especially sensitive to changes in salinity or pH.

Either shrinking or ballooning in size can be an early warning, though they can do either even if nothing is wrong. Later stages are loss of stickiness on the tentacles, loss of footing, and foul odor.

That doesn't sound good. How long have you had it? Has it done this before?

Does it feel sticky? Does it respond to touch? Do pieces fall off when you touch it?

Anemones will appear to be melting when they are dying. Stringy stuff will be coming out of the mouth. When all else fails, you can pull it up out of the water (if it's possible to move the rock it's footed on) and smell it. If it smells like foul rotten fish rancid garbage, then toss it down the toilet.

I thought mine was dying, it was very shrunk in for a few days and just looked odd. Then one day after work I had two.

Mine shrinks every night 1 hour before my daylight T5s turn off. Then the actinics sty on for 2 more hours and the whole time is stays shrunk. I guess its just telling me it sucked up enough light for the day. I plan on reducing my light cycle by 2 hours eventually.

My condy poops white stringy slimy looking stuff too. may be nem poop thats sticking to your fish tail.
