Always 4 weeks minimum. If I see any problems during those 4 weeks, then the 4 weeks starts over after the issue is resolved.
Um, I have to admit I don't. I don't know if it's necessary or not, but I've never had an issue. That's not to say quarantining shouldn't be done, just that I've never done it, nor have I ever encountered a problem.

Personally, I think its better to not because less stress and less changes in chemistry...
But i have heard of red bugs catching a ride in on inverts

Everything you plan on introducing into your D/T should be quarantined for at least 4 weeks. Inverts, fish and even LR have a propensity to introduce something you don't want in your D/T. And, as Reeron suggested, if something does materialize during quarantine, at least 4 additional weeks after the issue is resolved.

i have never quarantined any of my livestock, if the place you buy them from has thriving reef tanks its usuaslly a safe bet the fish are healthy. but fish from petco or other chains should get quarantined

I strongly disagree with bvb-etf-luva. IMHO, your playing Russian roulette with your livestock when you don't quarantine all new additions into your tank. Some outlets may utilize better precautionary methods than others, but none can absolutely guarantee you will not introduce a parasite into your tank. A quarantine setup is a relatively inexpensive investment you can make to ensure the safety and well-being of all your livestock.

but i have one more question: do you guys keep your quarantine set up 24/7 to keep the biological filter stronger or do you only set up when its time to add stuff
