Mine isn't too close to the surface & isn't pulling a vortex that would be an indication of it sucking air.
No blockage but what about an air bubble? That'll also slow the flow of water from the tank.
I have the same problem & I'm using a sponge filter over the intake which should break up any bubbles before they would enter the intake.

Make sure both ends of the siphon tube are covered with water. If one isn't you'll just keep on sucking air.

Good possibility that if you had the UV the outcome may have been different depending on what the COD was. UV is definitely a good thing.

Does your tank have an overflow box? You want to skim the surface to remove oils, slime, etc that usually floats on top.
