You should remove the crab before it decides your corals are food.
Getting the PO4 down and the Mg down will help. High levels of Mg can cause bleaching of corals.
I think something must have damaged that polyp. If you keep the water quality up and the coral fed it may recover.
I would think too much light and/or too much love from the clown.
Those things happen at times and are hard to figure out when they are localized. A guess would be that it got zapped by a pocillopora doing a polyp...
