I would point both returns at the surface. Try to get the surface to be quite rough looking.
It is having problems with its gills if it is doing that so oxygen could be a problem for the blenny. It sounds very stressed. Turn out your lights...

That is normal for Anthias to hide for a while until they acclimate to your tank.
They are far too strong for moonlights, you want a couple .5W LED only. They eye adjustment is normal also. I would buy LED designated as moon...
If the tank is heavily planted you will need that many, they don't move between tanks the best.
