Do you have your tank grounded? I have had a very similar issue in my new tank, I have lost everything except my lawnmower blenny. My LFS came to the house several times did multiple tests and even checked my sump system to make sure everything was working properly. the one thing they noticed was I didn't have a grounding probe in the tank. They gave me one and I havev't lost a thing since. I my issue I was only losing fish the inverts weren't being effected. And, typically they are the first to go unless it is a fish only disease.

Sounds like you have been busy. Sorry to hear about the loss of your coral beauty. I would agree as well as to the O2 issue. It's always important to have good water surface agitation, in addition to another powerhead/return blowing across your rocks/sand to keep from going anaerobic. In regards to the potential ich issue, I would just continue to ensure water quality is good, supplement the fish food with garlic and selcon, this will help to build the immune system of the fish. * Do you have a UV sterilizer on the tank?*

Sure. There are various Garlic supplements. I use and like Brightwell Garlic Power
    Brightwell Aquatics Garlic Power
Place aprox. 8 drops of garlic on your food, let soak for about 5 mins and your ready to go, the addition of Selcon would supplement a wide array of vitamins and minerals.

Just a word of caution. Be careful with "medications" for ich etc... Some of them contain copper which can be very detrimental to a reef aquarium.

So I checked on the marinoopharm and it is copper free. I wasn't able to find any other info on it as far as people using it so I can't comment on its effectiveness, but copper-free is a start.

Sounds good. If you are going to try to use this product err on the side of caution with dosing. Always start with 1/2 of the suggested dose and after 24hrs if everything appears ok, then put in the remaining half.
