I had the same problem with my two, I ended up moving the OB to a back corner out of the way were the BT does not realy go it took a few months but the OB started to perc back up to its old self .
BTs are a regular ofender of attacking OB
    hope that helps.

sadly i think alw is right, the tang won't stop if it now views the brain as food.
the only suggestion i could give you is try making sure the tang has plenty of nori to eat

I feed dry algae, brine shrimp with spirulina, myssis, squid, plankton and some times pellets. If a move the OB to the refugium and I put him back in a feew weeks?

Note: I have anothe OB and he is ok by now.

Try puttting several long leaves of lettuce in there and see what happens. Once they realize that is food I bet he will leave everything else alone. My blue tang eats an amazing amount of lettuce per day. I think they are a very high energy fish and require more food than most people think to be happy.

I am not joking here....Maybe it's time for a nano. And the OB can be the first coral in it. A little 10g would suffice and it could double as a frag tank.

Broccoli is far better then any lettuce or Nori I have use Broccoli to help reverse the damage of lateral line it works better then any food or medication I have ever used or seen. It will last day and not break down in the water and will offer far more nutrient's then lettuce . I allow my tangs to pick it to the nub and it helps to keep them mentally distracted and busy and entertained and preoccupied after the tang is like a cow they move and graze all day long its their Job.
