heater sticks on and nukes tank... always a heater.

 

VERYYYY large amount of food dumped in tank going unnoticed until it's decaying and ammon. spikes high

Improper cleaning of.. well.. anything and it causing an ammonia spike.
Bio balls, can filter, etc...

Putting contaminated hands in the tank.

 

I vote for the classic anenome in a power head. Instant tank nuke.

 

Letting a foam filter fill up with too much gook, then the return flow is too fast for the clogged filter to keep up with it, then the motor gets burnt out in the sump.

Or you forget to do your top-offs and your motor gets burned up in the sump.

Don't fix it in time and BAM...no oxygen for your fishies.
