    VERYYYY large amount of food dumped in tank going unnoticed until it's decaying and ammon. spikes high

    Improper cleaning of.. well.. anything and it causing an ammonia spike.
    Bio balls, can filter, etc...

    Cucumber dies and nukes the tank, no cable drip loops and theres a fire.

    or your doing a waterchange and accidently fill it with gasoline and later that night theres a power outage and the janitor lights himself a match and your cooking fish sticks! lol sorry I couldn't resist... YouTube - fire marshall bill

    Letting a foam filter fill up with too much gook, then the return flow is too fast for the clogged filter to keep up with it, then the motor gets burnt out in the sump.

    Or you forget to do your top-offs and your motor gets burned up in the sump.

    Don't fix it in time and BAM...no oxygen for your fishies.
