    Some guy put a chiller in an unvented cabinet, fire started, burned down part of his house.

    Return flows from sumps and fuges should rely on gravity. If the intake to the fuge or sump fails you won't get an spill.

    Accumulation of detritus, escpecially in sump. Lack of maintenance, care, husbandry. Tank neglect. As said, neglecting mechanical filters.

    Anoxic pockets develeped therein containing noxious deposits of hydrogen sulfide are disturbed, liberated, and wreak havoc on the tank.

    In the winter, heater not large enough, or stops working.. Temp drop.. Stuff dies, then crash..

    Almost happened to me.. Here we don't have much cold weather..
    This last winter, I had to place an elc. heater in front of my tank a few nights to keep temp right..
    My heater wasn't up to the job.. So had to help it. Now I have 2..

    had the same thing happen. lost a ray, lutienent tang, clown tang, and powder blue tang, plus a cleaner wrasse... :-/

    as for the list...
    given wrong information when making a purchase or not having all the information when making a purchase (tank was really ran w/copper, that fish is definately reef safe (not)...)

    Seems to me the biggest killer of tanks is power outages, and not having a back-up plan. I.E. generator etc.
