My false perc is growing green hair algae on its stomach and swimming like a mad man in circles. He refuses to eat and generally just looks unhealthy.
All params are good except nitrates at 10.

I have never heard of anything living having GHA growing on it. Do you have a photo of it? It might be a fungal growth on it. It's hard to say what it is without a positive ID on it.

There is also some white mucus on the side of my clown with a small chunk taken out of his fin. I have no clue what could have caused this.

the Green stuff is right under the fin and streams along the side as he swims.

Could it be a parasitic worm(s)?

The clown has a severe infection. The algae is most likely growing on the dead skin. But, could it also be having trouble passing a stool that is mostly algae? Kind of looks like it could be from the same area.
Either way I would QT the clown and start anti-bacterial treatments. Target feed it mysis fortified with garlic, selcon, and vitamin c

added garlic to a mysis shrimp frozen food and target fed him and he didnt even budge. He is sitting on the bottom. Im not sure this guy is going to make it. His fin seems to be rotting by the hour.

I doubt that it is actually growing on him. Probably stuck on him somehow.
