    Sorryto hear about the Tang!
How long have you had the Yellow Tang?
It may have been sick before you purchased it!
Other inhabitants in the tank that may have quarreled with him?
Any other fish look sick or infected with a disease or other pathogen?
I wonder if you have a low level of dissolved oxygen in your water?
Unfortunately, many of the problems we encounter are iatrogenic(caused by human error oir carelessness)...not to say that this is what you experienced, just a stated fact!
Changes and shifts in your water parameters can also play a major role in fish and other animal health!

i got the tang from a person who was breaking down their tank so i dont think it was either sick or under shock, the only this that i have changed in my tank lately is adding a sump, would that affect him? also thus far all other fish seem to be fine

usually scavengers will eat @ the soft tissue first, hence eyes missing. same as land scavengers

what other fish do you have , IS he the more sensitive fish in the tank ?
Could it have been a polutant from introducing the said sump.
Were his eyes missing befor he died?......sorry just kidding lol Laywer talk.
Are your heaters working ( no cold snaps over the knight/ windows open?)
Had he been feeding as normal or seemed unhappy or depressed?..(If you could tell..hehe)

My guess is that he may have been an older fish and just gave up the goit happens
    sorry for your loss
