honestaly thats the first thing ya need to test befor going any further but other things to test are nitrite and keep up on the ph, salinity, and sometimes fish pass without warning. firefish are "jittery" anyway and may have gotten stressed from the tank mates. I would wait a few weeks to two months before adding any more fish just to be safe.

    What is the current:

    PH
    Nitrate
    Nitrite
    Ammonia
    Temp.

Those levels are the important ones when a fish dies for no reason.

IMO, 5g every 10 days in a 28 (factoring in the space that LR/LS takes) is a little overkill unless there is no skimmer or carbon running, among other things. It certainly won't do harm but what is the point?

Ever hear anything clicking in the tank late at night? Mantis shrimp make easy work of a firefish that is hidden in between some rocks. Doubtful that is the cause but something to consider.

IMO calcium is a bit high and you shouldn't have any sign of ammonia.

I also bought 6 tests in One Strip Quick Dip just to see how accurate they were. Was just curious how they worked. Seemed pretty close to all my other test results.

I would take some water to the lfs and have them test it, compare the results. Typically I test everything, record the results and bring that to the lfs.

And again sorry for your loss.
