    Did you have him quarantined in a seperate tank or introduce him straight into your display? What kind of steps did you take to acclimate him to your system? (temp, pH, salinity, etc)

    I'm not suggesting that the fish wasn't sick or unhealty before you got it but I would imagine you can easily make a bad situation worse by stressing the fish out more than it already is.
