The spots are about the size of a pin head. If it's not bad, you may not notice anything different. If he stops eating and is covered with them, then I would quarantine him and treat with a copper based medicine.

The tank is at 75deg. I did not put him/her in a quarantine tank first. He/she was at the LFS for 2 weeks in a copper treated tank (they treat all their fish only tanks w/ copper). I have a 29 I could move him to if I could catch him. The only thing is last week I had a heater break (it still was on) in the 29 and my entire room had a metallic stink. There was a section of sand burnt black with glass shards in it. A titanium heater is on its way I don't know if it would be in his best interest to quarantine him/her, if I could catch him. Are there any reef safe ich meds? If fish have ich, do they twitch, or shake? He/she is not rubbing on anything. If I raise the temperature and do do regular large water changes could he get better on his/her own?

IMO its tourette syndrome. You should seek therapy for it. If it starts swearing too its a definate diagnosis.
