Definately flatworms! I had a bad infestation and all I did was to cut way back on my feeding and they ended up killing themselves out in a couple weeks. But you can buy actual medication for a flatworm problem.

Whether or not they will die off on their own depends a lot on the species. The red planaria tend not to. Here is a good reference if that is the case (I can't see the pic well on my phone)

Thanks for confirming that they are flat worm. The more I look... the more I find in the tank. I see that they will release toxins when killed. Is there any fish or inverts that will eat and keep these suckers under control?

some wrasses including 6 line and I know theres some others some may chime in on.You can also siphon as many as you can with airline tubing which can help make a dent in the population.I've never had the red ones, but have heard a mass kill off can be bad for the tank.

The infestation that I had was the exact kind you have in the pic, the red kind. And starving them out by not feeding the tank worked for me! I had no ill effects of the numbers ging down because it happened slowly over the period of a few weeks. But some wrasses do eat flatworms, just google it, you will find something.

My tank is a 10gal nano
    2 ocsellaris clowns
    1 blue legged hermit
    1 left handed hermit
    3 blue knuckle hermit
    8 margarita snails
    1 turbo snail
    eagle eye zoos
    purple zoos
    anthelia waving hand
    devil's hand leather
    kenya tree
    green striped mushrooms
    red mushrooms
    blue mushrooms
    Large frogspawn

I had em like that too....I used salifert flatworm exit. siphoned out as many as I could. they start dying instantly and start to float around the tank. I did an immediate large water change within 20-30 minutes after dosing to remove some of the toxins they release.. I have done this twice and everything as survived each time. I was nervous of a tank crash, but it all worked out in the end. GOOD LUCK!!!!;D
