What are your water parameters (ammonia, nitrite, nitrate, temperature, salinity). What is your tank size, and what filtration do you have. When was your last water change, and how often do you do water changes? How long have you had this wrasse? Is it eating? What are you feeding it? Does it have any obvious lesions (white spots, ulcerations, etc.) you mention white patches - are these tiny spots (as in crypt), velvety spots, or just pale large patches? Please do your best to answer these questions and we can help you better.

    Update, Picture

    Temp- 77-79
    Salinity - 1.024
    Ammonia- 0
Nitrate- barely shows on test.. 0-0.5

    Ehiem Canister filter

Tank Size is 46 Gallons, Ive had the wrasse for 3 months.. recently its appetite has been slowing down..hardly eating at all now.. I feed it Omega one Natural protein Formula Marine Flakes, New life spectrum sinking pellets and marine fusion frozen food mix, its got krill mysis brine shrimp, mussels, squid and algae & Hufa.... in frozen cubes.

20% water change every two weeks, Next ones on the 1st.

and the picture shows a bit of .. what i can only describe as a faded patch.. of white? behind the gills there.. and on the other side nearest the tail

It's hard to see the white patches from the pics, but I'll give you my opinion anyway. I don't know what's wrong with this fish (ie. what is causing the problem).

I would suggest feeding it vitamin-enriched meaty foods (wrasses are mostly carnivores). So I'd take some frozen mysis and soak it in a vitamin supplement like selcon and feed it a few times per day. Keep your water quality high, and most importantly, stable (don't allow any swings in salinity, temperature, alkalinity, etc.).

I would not remove this fish to quarantine at this time, as that would surely stress it further. And since I don't know exactly what the problem is, I would not recommend any medication at this time.

One last thing - make sure none of your other fish are bugging it or nipping at it. Also make sure it has a pile of sand somewhere in your tank that it can bury itself inside of - that will make it feel safer, as most wrasses like to bury themselves on occasion.

    Keep us updated.

Sorry I cant help ID the problem Zegermans 13

but can you tell us what else is in the tank with it - fish and inverts

question - do you have any kind of starfish in there?
as I have heard of examples where a starfish has found a sand sleeping wrasse and caused markings on it body trying to envelop it to eat it

    Bingo

the brittle star has found it in the sand and enveloped it during the night
causing the wrasse stress so not eating and causing those lessions on its side IMHO ( I believe it happened to me, thats why I asked the question)
