Its hard to tell from the picture by I might guess Pop Eye. Get some Melafix. Ive had a Blue Tang for about a year and he at one time came down with Pop Eye. If you dont want to tear your tank apart that is fine, treat the entire DT. Reef safe

i'd also keep an eye to see if the affected area continues to erode along the lateral line to ensure its not HLLE as Corailline mentioned. It could be in early stages.

My reason for thinking this may be a possibility is the fact you mentioned that he doesn't seem to like nori. I could be wrong, but I believe that not enough greens in their diet can contribute to this condition.

If he is still eating, you may also want to try soaking the nori in garlic to entice him to eat it. Just a thought though...

in your description you mentioned that it's eyes are bulging and cloudy. i would also guess pop eye.
i am currently treating my yellow belly tang for pop eye and melafix as Ryland suggested didn't do it for me so i quarantined it and used Furan2.
you might wanna try and quarantine your fish and use furan2 if it is pop eye. mine recovered in just 2 days.

Here is what I did with mine when my tang shows some sign of ick. I fed my tang with diff kind of frozen food so I decided to melt them all in small container (brine shrimp, squid, nori, pellets etch) and add a small clove of completely mash garlic, mixed them all then I freeze it. Once it's frozen I grab little and feed the tang. After a couple of days all signs of ick are gone. Try it I guess fresh garlic really helps.

Thanks everyone. I have anemones but nothing recent except some zoas. It seems to be getting better on its own but I'll continue to monitor it. We'll see what tomorrow brings.
