I am having my first ever outbreak of marine ich and am setting up a hospital tank! I was wondering if you can use a freshwater setup for this. What is the basic necessities for a hospital tank?

A basic setup will do fine for a hospital tank. Heater, a few rocks, HOB filter would be fine.

Keep in mind though that you will have to treat all of your fish (even the ones not showing symptoms) in the hospital tank for 6-8 weeks to completely rid your display tank of the bug.

Or, you could keep the parameters of your display tank in check and soak the food in garlic and let the fish fight it off themselves. I have had success with this in the past.

    Good luck,
    Ivan

Bare bottomed tank, filter, heater, water flow/aeration will suffice, no need for lighting and if you want to add a piece or two of dead rock, that will be fine. Proper medication and dosing is crucial!

Rather than rock use non porous things like PVC pipe or fittings so they can be disinfected and reused. Rock would need to be thrown away since it will absorb medications.
I keep my QT and hospital tanks very simple, a bare bottom tank, the appropriate size AC hang on back filter, a heater, a powerhead if its needed and the correct lighting for the specimen being QT'd or medicated.
Everything can be cleaned and disinfected easily and used again.

Just try not to stress your fish during the transition. With ich the more stress the worse it will get. Make sure the quarantine tank parameters are close if not identical to the DT before transferring the fish and depending on the size of the QT you may only want to keep one or two fish in that at any time. Also I would recommend a clean freshwater "dip" before placing anything back into DT. Some medications have chemicals that can be disastrous if even a small amount reaches your DT.
