Seeing as, to my knowledge, they only eat small( or sometimes large) starfish, how can i tell if he is well fed?

first off, save your money and dont buy chocolate chip starfish or any other big starfish. Feed them asterina starfish. These are the little white starfish's that you can find in an aquarium. I went to a fish store once and would just ask them if I could take them off their hands. Got them for free. Get enough of them and they will multiply, that way your harlequin will always have food. Mine would run around the tank, he was funny. To answer your main question, if he isn't dead that's usually a good sign. You might not see them for a couple of days but then out of nowhere they will pop back out. If you see them, they are healthy.

a quick addition, when you put in a starfish, big or small, the harlequin should start going directly toward it. I would drop an asterina star on the opposite side of the tank, and he would walk straight over to the other side and grab it.
