I had one for about a month, he was really cool and he would eat the sides of the glass where algae was. Think it slowly starved to death you probably dont have enough pods to support him?

Lawnmower blennies are omnivorous. But their main diet consists of the green stuff. I got my blenny eating everything until it disappeared too.

There are so many things that could cause a fish to die it's hard to say.

STRESS - abrupt temperature swings, ph swings, salinity swings, being picked on by other fish

DISEASE - any spots on his body, cloudy eyes, damage from handling or other fish

AGE - do you know how old he was? Was he his full adult size when you got him?

Unfortunately sometimes you can't pin point why a fish goes but you can think about some of the above to see if there was anything within your control.

im pretty sure he was his adult size when i bought him from petco. and he never really seemed to adjust to the tank, he would always hide when any one would stop and look at him in the tank.
