What are the water parm? Do you have a UV? I had a couple off spots on my Royal Gramma and started soaking his food in garlic and he cleared right up.

What did you do last time? If only garlic, you introduced the purple to a tank already full of ich. What else do you have in the tank?

If I were you I would take out at least the purple and quarantine it, but a better option is to get out all fish or treat the entire tank.

first i have 2 blue chromies, a psychedelic manderine, a sixline wrasse, 2 ocelaris clowns the hippo, purple, pygmy angel purple with yellow face, lots of coral, around 60 different types, some frags some big clusters, 100lbs of different rock, everything, everything is always great, everything opens and feeds, water is usually pretty good, no sterializer, its a 75 with another 30 below between sump and refuge. only problem really is ich with the tangs, maderine got it once, gone in 2 days. getting ready to move to a 125 long within a month or so.

also last time i dropped the salinity a bit raised the average temp to 81 and over fed garlic flake for about 10 days then changed about 40 gal of water and brought the temp down slowed feeding again, and the hippo has never been beter. allthough i did still lose the powder blue, im rebounding with the purple and thats all thats in shock, now the pygmy chased him around for a good half a day when he got there but thats stopped no bullying at all from anybody, rearanged a little rock as well

I just soaked the food in smart reef garlic, I used reef veggies or pellets even frozen brine. I don't think garlic is any sort of miracle cure for all fish diseases, but rather an appetite stimulant. This is something that has long been known by sportfishermen. When a fish eats, it's immune system is stronger which can help it fight off diseases and infections. So garlic isn't a "cure" but can indirectly improve overall health.

I would say you have too many fish for a 75. Adding him or her with that many other fish is way too stressful in such a small space. The ich is going to always pop back up under that stress. QT tank by itself is best treatment. You can't treat the ich with that many corals.
