I hade a 4 stripe damsel, it was acting pretty normal. One day I saw him dead on the sand, I was shocked, their was a hole around his gills, and his fins were all messed up. I have 1 blue leg, 2 green leg hermit crabs, and a cleaner shrimp. He wasnt old, and I tested the water at the lfs and it was fine. Any ideas

I would think with the other inhabitant of the tank that the damage was done post mortum. Was it eating well before the death? was it flicking on rocks? Was it spending all it's time at the surface?

Some times these things just happen.

    Sorry about your loss
