I have an Orange-shoulder Tang and I see small pepper spots on his body. Is this a defense or camouflage? Trying to get a clear pic for you. He is in my QT tank. Nitrates are around 15 in the tank because I like to get my fish thick and healthy before I place them in the DT. I perform 20 gallon water changes weekly when I have my fish in quarantine. Any thoughts?

It is a dry heat, yeah right !
    Could it be Black Ich?

    Tangs are famous for it.

Actually the spots are little spots of blood where the worm has been feeding.

LOL....we were posting at the same time.

Even though it's called black ich, it's not ich at all as Cheryl pointed out. They are flatworms. Yes, it is definitely worse than Ich. Everytime the worms drop off the fish, they reproduce and then more go back to the fish the next time.

You need to QT the tang and give him a methylene blue dip in freshwater. Then you need to do a formalin dip afterwards.
