Looks like my rose tip anemone a few months back before it died. I have to ask what are your parameters, has anything been hosting in the nem, is it still hosting, what feeding schedule do you have the anemone on, and what kind of lights do you have?

If it isn't sticking to anything it may be looking for a better spot in your tank flow and lighting wise. Also I ask about your lights because I run PC's on my nano and find that anemones will just slowly wither away under them.

If your anemone is dying it will start to come apart in a loose taffy kind of way. I think once it does this it is just gone. Just something to be aware of

    Welcome,

When did you buy the anem? How are the params in your tank? Lighting?

It could just be expelling waste. How often are you feeding it? I think overfeeding, meaning more than 3-4 times a week, is more harmful to an anem than underfeeding. Also, about it not sticking to anything. It may just not like where you put it in the tank. The flow may be too powerful or the lights might not be right. I know my anem moved all over the tank till it found a spot it liked.

EDIT: Also, about two weeks after I got the anem he let go of his position and got picked up by the current in the tank and floated into the business end of my koralia 4 powerhead. His foot got a nasty tear in it and wouldnt stick to anything for quite some time. Its been 3 months now, he's made a full recovery and is stuck to a nice rock right now. And the powerhead now has a foam piece attached by a rubberband so this wont happen again. So to relate this to your question. The foot may be injured. And also watch your powerheads if this is a new addition

In the event you decide to get another nem, read the following article. It was referred to me by an experienced reef enthusiast from this site.

    ANEMONE CARE AND TIPS

know that (most) captive anemones have an avg survival rate of less than 12 months ....thats being generous.
