    Can you give params, tank size, filtration, and livestock please?

    All levels are perfect, 30 gallon tank, hob filter with carbon, bio-cube protein skimmer, there is a clownfish, a conch, a few crabs, a frogspawn coral, and 15 lbs of live rock

    temp is 81, salinity is 1.026

    First that scopas tang is WAY to large and needs far more swimming room than a 30 gallon.
    It could be Hole in the Head, what are you feeding it? And actually list your params please to.

    Im feeding it a type of algae flake food, i will list the parameters later. What do you mean whole in the head?

    Ive had the fish for 2 months and the tank is 7 months old, it was pre established at the pet store for over 5 years, then bought the whole setup and moved it to my house.

    If the tang is still eating I would get some marine algae sheets and better quality food than most flake. Selcon, garlic or HUFA enhanced food would be a wise choice to add as well.

    You say all parameters are fine but I would double check them. Make sure no other fish is bully the tang and think about a bigger tank down the road.

    It's probably stressed by the tank size if nothing else. They need 75+ gallons.
