Snowflake eel losing tail, mouth!!

Hey everyone, this is my first post and am in bad shape. My snowflake eel hasn't been eating for over a week, and now I have noticed that his tail has begun to fray and the area below his mouth is basically gone, like there is an outline of his jaw and there is a hole in his throat.

I think it is too late to save him but what happened? Has anyone seen this before? If anyone can help I would greatly appreciate it, I've had him ~2 years and I'd say he's nearing a foot long..

 

is there any way to get a pic of the damage? also what are your parameters / tank size and what else is in the tank with the eel?

What have you been feeding him ?
Sounds like it could have been a parasite type infection, maybe from live food.
