The wild adventures of Mrs. P

So I introduced a sixline wrass and a Midas blenny(named Mrs. P) yesterday. Everything was great for the acclimation and introduction to my tank. Both fish we're getting along with my two clowns famously and both new additions ate all of the mysis shrimp I fed them. All was well and the lights went out and the fish went to bed. I woke up at 4 am to be at work by 5 and with the lights out everything seemed calm and normal in my tank. My girlfriend called me around noon and said she could not find Mrs. P anywhere. The sixline was out swimming around with the clowns but Mrs. P was nowhere to be found. I told her to throw some mysis in and maybe she would come out(she was probably just hiding out in a hole in a rock somewhere and needed a little encouragement to make an appearance.) Still no sign of Mrs. P. I told her to look behind the tank and all around the tank because maybe she jumped out. I wasn't too worried about this because I put eggcrate over the top of the DT and there is no way she could have jumped through. Still no sign of her. I told her not to worry and I would check it out when I got home from work. I finally made it home and searched the tank with no luck. No I started to get worried. I was dreading finding the carcas of my brand new fish somewhere in the bottom of the tank getting mauled by snails and crabs.
    Still couldn't find her.
I got discouraged and decided to do my daily tank maintenance and opened the stand to top off the sump with RO/DI. I clicked on the light and to my surprise I found good old Mrs. P hanging out in my refugium. I grabbed my net, scooped her up and threw her back in the DT. Fed her some mysis and now she is happy and swimming around like nothing happened.
The question I pose is how did she find a way through the 3/8" gap between the top of the overflow and the eggcrate, swim through the drain pipe and make it into the refugium (there is a y connection in my PVC plumbing that takes a line to the refugium and the other to the skimmer part of my sump)?
Should I be worried that she might do this again?

so happy to hear you found her! I'm not an expert by any means, but I'd have to think she could do it again, but hopefully won't :)

Eggcrate is too large to stop most smaller fish unfortunately. You should replace it with clear mesh that Bulk Reef Supply sells and never have to worry about it again.

It's not the eggcrate she got through. It was the very small gap in between the eggcrate and the top of my overflow. My locline prevents the eggcrate from sitting directly on the overflow. I built a screen to go over the overflow and put "jailbars" up to prevent this from happening again.
Good news so far. She didn't go on the adventure last night and is happily frolicking in the tank today.

Oh ok. However my point is still valid. Eggcrate should not be used to prevent fish from jumping :-/
