Most times ich is a direct result of stress brought on to your livestock! Plus with the numerous times that you are adding fish to your tank just compounds and increases the chances of ich or marine velvet taking hold.
A quarantine tank will help plus going slow with the introduction of fish will help along with good water chemistry(temperature, salinity, pH, alk. etc.)!
Not to brag, but the inhabitants of my 125 gallon and 30 hex have never developed ich! That is over 5 years with no ich what so ever! No medications, uv sterilizers, witchcraft or other potions!
My Yellow Tang is notorious for contacting ich like a magnet, but mine has never gotten it since I bought him five years ago!
I guess, I am trying to point out that the reduction of stresses and consistency is key!
Go slow and read to help you as well as ask for help here on 3reef!
Sorry for your losses and hope good luck comes your way!

I'm just not sure how they got so stressed. I never added multiple or large numbers of fish at a time, and always made sure they acclimated well and were eating before I added more?

All fish have ick, its the slime coating that protects them from the outbreak. What keeps the slime coat? A healthy diet and of course, no stress. BTW,those little anemones are not good, they will sting other corals and eventually kill them. Get rid of them with lemon/lime juice or boiling water with a syringe.

Welcome to 3reef. Just soak you food in Vit C or garlic and it will help keep the ick down.
